Anticoagulation Clinic

The Trinity Health Anticoagulation Clinics are supervised by pharmacists who are certified in the management of anticoagulation therapy. Anticoagulants are medications that help prevent blood clots. Although they are commonly referred to as blood thinners, anticoagulants (such as Coumadin®/warfarin) are used to slow the amount of time it takes your blood to clot.

Most of your appointments will only be about 15 minutes. However, your first appointment may take as long as 60 minutes. Your first visit will include a review of food, medication, and lifestyle interactions as well as common side effects of Coumadin®/warfarin therapy. You will also get your PT/INR results, dosing instructions, and answers to your questions and concerns regarding Coumadin^®/warfarin therapy.

Our Services

We offer the following services to you and your physician:

• We monitor your blood using point-of-care PT/INR testing which helps determine the amount of Coumadin^®/warfarin required for effective treatment. PT/INR blood testing is obtained via the finger-poke method and not through the arm.
• Your result will be provided within a few minutes of testing. This allows us to easily and immediately adjust your Coumadin^®/warfarin dosage during your appointment.
• Services are provided by appointment only. No more waiting at the lab to have your blood drawn. You will be seen at your designated time.
• We will work in collaboration with your physician and a summary of each of your visits will be forwarded to him/her.
• You will be provided both written and verbal instructions and information.
• We can call or fax Coumadin®/warfarin refill requests to your pharmacy.

Referrals

First, discuss with your physician whether or not the Anticoagulation Clinic is right for you. If you and your physician agree that you could benefit from our services, have your physician contact us.

The clinic team will work closely with your referring physician and other health care providers to ensure the safe and effective monitoring and management of your anticoagulation therapy.
